Identifying hormone imbalances

Hormone testing and evaluation varies between providers and clinics. Between hormone blood, saliva and urine testing, which tests are best to help you identify a potential hormone imbalance?

As a naturopathic physician and a functional medicine practitioner, I rely on comprehensive testing to help guide my decision making and treatment plan. I also don’t rely on just one set of labs. This is a sample of a test that I use in my practice to help identify potential hormone imbalance. The hormones that are tested include:

  • Estrogens

  • Progesterone

  • Testosterone

  • Estrogen metabolites

  • Adrenal function

  • Cortisol

  • Thyroid metabolites

A very important point to consider is that this test is great to do when first evaluating hormone levels. When you start doing any sort of hormone treatment whether botanical, glandular, supplemental or bio-identical hormone replacement therapy, you can use other forms of testing such blood. In terms of saliva testing, I only use this form of testing when testing cortisol only. Although urine testing is not known to be the standard for testing thyroid hormones, I have compared both urine and blood thyroid levels and they seem to correlate to each other. Furthermore, a blood test may show that the levels are fine (while thyroid hormone fluctuates throughout the day!), but the patient will present with classical symptoms of thyroid dysfunction which the urine test can confirm.

Signs of a hormone imbalance include:

  • Weight gain or weight loss

  • Hair thinning

  • Increased adipose tissue around the abdomen

  • Low libido

  • Fatigue

  • Low motivation

  • Excessive sweating

  • Sweet or salt cravings

  • Infertility

  • Sleeplessness

  • Wired but tired feeling

  • “Second wind” at night

Hormone imbalances can happen with age, due to prolonged illness, sudden weight loss or weight gain and also due to environmental causes including work/life imbalance, toxin exposure and poor nutrition.
