How to schedule an appointment:
We recommend signing up for our initial 30 minute visit in office or over the phone. During this time we can learn more about you, discuss what you have already tried, and the best course of action we need to pursue.
Billing and Insurance:
Proactive Health is an out-of-network provider and does not accept insurance directly. All payment is due at time of service. Be sure to check with your insurance company if your plan includes out-of-network providers. You will be provided with a coded superbill to submit to your insurance company for reimbursement.
